Effective date on name change

It would be useful to have an effective date field on the name change form. Currently there is no way to set the effective date of a name change.

End terms of employment when terminating worker

With personnel actions on, when worker is being terminated, user can decide whether to end benefits, compensation and leave and absence. Please either add terms of employment or end it with the employment automatically. Thanks
