When workflow is enabled, employees are no longer able to upload a certificate. We would expect the next behavior: 1) Employee adds a certificate adds the certificate to the flow 2) N+1 receives the approval step and review the document 3) N+1 approves and immediately the document is attached to the certification.

Totally agree. How can a certificate be approved without an attachment ?

I am not sure that adding the document as a part of the workflow is what makes the most sense to me. I think the user experience of adding a certificate for example is not intuitive enough for users to know that they should click the attachment icon to add the attachment. I think a UI update and a way to indicate if an attachment is required would fix this problem without making it more steps and difficult. Perhaps by adding a flag to the certificate type itself to indicate if an attachment is required, then adding a button to the action pain that is more visible and obvious called attach certificate. Then validating if a certificate is attached before you close/submit the record.

I would say it is not a bug that you cannot edit a record after it is submitted to workflow. The current process would be to put the workflow back in draft, attach the document, and then resubmit. This could likely be fixed with some training as well. I do see value in making the UI easier and more obvious that you need to attach something and where to attach something.
