The anonymous response switch is automatically set to "yes" following any changes made in a Customer Voice survey

If a survey is generated in Customer Voice with anonymous answers set to "no," the survey will revert to "yes" as soon as any updates are made. This ultimately enables anonymous survey responses.

It's a recognised issue by Microsoft where the switch can be activate when there are changes made to the survey.
