Having a same template for any review of an employee seems redundant and doesn't really help to provide an insight to an HR Manager when they're looking for turnover reports etc.

I would like to suggest to change the form for exit interview in the Employee Review to something more specific that allows an HR Manager to retrieve information that is useful. For instance, the form should have drop down options of reason why they're exiting that should be customizable.

The system also should generate a report where the HR can visualize why workforce is leaving the organization. Normally, non profit organizations don't have the budget to get Power BI license so it would be helpful for them to utilize something out of the box.
