[Prerequisite]
1. Provide [Voice of the Customer app access role] role to Global admin or Dynamics 365 (Online) system admin.
2. Open VoC app on UCI and create Survey on designer screen.

[Demand]
3 items are added when deploying radio button or check box on Survey.
It's unclear how to add more than 4 items.
It would be much better if [Add] button on the same line in [Clear], [Copy] and [Delete].
