We figured out recently that 2 agents accessing the same case record at the same time (as they work as a team) can both edit it. Hence, one of these guys' changes are lost forever. That sounds a bit of a joke for a system of this amplitude. What about locking records when someone first accesses it and unlocking it when he leaves ?
