If adding a Note or attachment on head level or line level of an Vendor invoice journal, General journal or any other journals, that attachment is not visible in Ledger settlement or Trial balance.

If you add attachments in Trial balance on transaction level it's visible in ledger settlement.

You should be able to review comments/attachments added in the whole flow.
