We have tested this scenario in std. environment by enabling and disabling the ‘Include both name & Description’ checkbox in the Form setup(Navigation: Procurement & Sourcing>Setup>Forms>Form Setup) and have observed that

In 1st case: when the ‘Include both name & Description’ checkbox is disabled we are not able to see the item Description whereas only the product name is visible in the Purchase Order>Line Details>General >Text field

In 2nd case: when the ‘Include both name & Description’ checkbox is enabled we are able to see both the product name and item description in the Purchase Order>Line Details>General >Text field

The customer’s reported concern is that :
The purpose of that check box is to populate both Item name and Description in the purchase order lines when turned on.
The check box says "Include both name and description". So, when it is turned off, only the description should flow to the PO lines.
You can further test the behavior of the check box by creating a Purchase order directly.
When turned off, only the Purchase requisition line description should flow - which is not working.
When turned on, both name and description should flow - which is working fine.
So, when that check box is turned off, it is not working as per expected.

Hence:

In 1st case: when the ‘Include both name & Description’ checkbox is disabled we are not able to see the item Description whereas only the product name is visible in the Purchase Order>Line Details>General >Text field
The label can be interpreted in 2 ways for the check box off – this could be show only description or show only name. it is implemented as the latter and the customer expects the first

In 2nd case: when the ‘Include both name & Description’ checkbox is enabled we are able to see both the product name and item description in the Purchase Order>Line Details>General >Text field
Yes in this case you are able to see both – and that is obviously also what is happening when PO is created from a requisition – that it concatenates the 2 information into 2 text field.

What we are missing is probably a 3rd option of show only description

Thank you very much for your input. We would like to inform you that this capability became available in version 10.0.12.
Follow the steps to activate new functionality:  
Activate the parameter that controls the item description transfer from PR to PO: Procurement and sourcing > Setup > Policies > Purchasing policies > Purchase order creation and demand consolidation rule > Item description transfer tab > "Always copy item description to purchase order" parameter in the "For non catalog items" section. 
Once this parameter is activated, while purchase order creation process from purchase requisiiton, the system will use the item description defined on the PR disregarding the form setup configuration "Include both name and description".
