App for Outlook doesn't work with On-Premise
After applying the December 2016 update I have attempted to set up the App for Outlook.
When opening the app I see the ADFS login screen appear in a popup window. I complete the login details but am then just shown a blank web page instead, and the "Having trouble signing in?" message is displayed within the app.
Using the F12 developer tools in the popup IE window seems to indicate there are JavaScript errors on the page (attempting to access properties of the window.opener object, which is undefined)

Pin App for Outlook
Users that track the majority of their email using the App for Outlook find it very time consuming and frustrating to keep having to click the "Dynamics 365" button in the Outlook ribbon to re-open the App for Outlook after selecting each email.
If there was an option to "pin" the app so it stays open between emails this would much improve the UX of this feature.

App for Outlook: Track regarding a record with ampersand in the name
Using the App for Outlook, track an email and set it regarding a record (account, opportunity, whatever) that has an ampersand (&) in the name. Tracking the item fails and shows a "retry" button, which fails again.
This seems to be due to the ampersand being included unencoded in the XML document that is posted to the Exchange Web Service. This should be encoded correctly as &
