We need a feature to get choice-type questions from a list.

This way, we can use that list on multiple surveys and update it easily.

We need the user to add one or more names to the form.

We need a pop-up so that they can add either one, two, or more names to the list.

Currently, we have to create multiple questions, so they would be able to add those names in different fields.
