Currently Customer Voice's survey designer does not offer the Date/Time picker question type.
User can only input a date but not a time or date and time (M/d/yyyy h:mm tt)

An option for a separate Time Picker question would work as well.
Need regional support for 24 hour time format as well as the old 12 hour am/pm for localities still using that.
Consider mobile device user preferences.
