After we upgraded to Dynamics 365 Online from CRM 2016 On-Premise, when opening the out-of-the-box form the 'Document Location' entity is throwing a Script Error - "One of the scripts for this record has caused an error. For more details, download the log file.", with the consequence that the 'URL Type', 'Parent Site or Location' and 'Relative URL' fields are not shown.

Even if you create a new form, same error is thrown.
