The 'Qualify' lead button doesn't seem to work on the Lead form. This problem only occurs for forms which were created from scratch. This behaviour does not occur in the OOB 'Information' and 'Lead' forms. This behaviour occured when we upgraded a client's CRM 2013 on-premise system to a CRM 2015 on-premise system.

The workaround for this problem was to merge the form in question with the 'Information' form and use the new form.
