Update Form Hosting Script to Bypass Adblocker

The super long script that I put on our website to display a contact form causes several popular ad blockers (uBlock Origin, Poper Blocker) to stop it from rendering. I have never had this problem with any other form tools and marketing automation systems (including Formstack, Marketo, Act-On, Pardot, etc.). The script is way to complex. Needs to be simplified and tested against these ad blockers.

At the very least, instead of the infinite circle of a form not loading, perhaps put a message that says "Your browser settings are not permitting this form to load. Please disable all ad blockers for this site." Or something like that.

It may be the dynamics.com URL that ad blockers don't like. So maybe just switch forms to a different URL.
