Lookup field not loading from second time on Quick Create form on Vanilla Org

Access - OP environment

Version - OP 9.1.21.5

Click on quick create

Open any table lets say "Person"

For the first time Magnifier is loading

From 2nd time no magnifiers are showing on the form.

Note : This functionality is working fine in UCI
