It is very clumsy when user needs to lookup many times for adding the lookup items in the same table.
Recommend to allow adding multiple lookup item in one go which similar to the "Adding Products" features for product line item

This is a basic and mandatory field and that should be out of the box everywhere for the lookup field in Dynamics !
