The idea is to have something like a Likert option which can the addition of more than one column and the choosing occurs not from an option set but from a text box which input is a numerical value.

To better explain the request is to:
When designing a survey, we need the possibility to add a new question which could look like a Likert but to also have the possibility to edit it by adding more than one column (at least two) and the possibility to input a numerical value against each row.
