Enable the Address Input component to be added to more than one address.

Whilst editing the Account form, I was only able to add the component on Address 1: Street 1 as it wasn't available for Address 2: Street 1.

Activity Screen - Convert To Opp pop up needs to be configurable

Currently, if you are on an Activity Record and choose Convert To and choose Opportunity, you get a pop-up screen that is not configurable. It would be great if this could be configurable as there are fields on it that may not be needed and also fields not on it that would be needed.
